Welcome to the Medical Staff Reappointment Process at Ontario Shores. Ontario Shores is now utilizing CMaRS for Reappointment Applications. You will need to use the link that has been provided to you to access your Re-Appointment Application.
Completion Date: Your online Re-appointment Application must be completed in full by JULY 15, 2023.
If you require assistance with the online Re-appointment Application, please notify the Office of the Physician-in-Chief (contact information provided below). They can assist you with the on-line application process and ensure your application is completed by the JULY 15th deadline.
If you are not applying for Re-appointment for the 2023/2024 year, please log in to CMaRS and decline the application and please send a letter of resignation of privileges to Julie Orchard, Executive Assistant to Dr. Amer Burhan, Physician-in-Chief at @email or 905-430-4055 ext. 6053.
Requests for any changes in status or privileges are to be communicated in writing to your Department Chief prior to the JULY 15, 2023 deadline.
CMaRS Re-Appointment
The online Re-appointment process consists of:
- Completion and submission of the CMaRS online Re-appointment form>
- Uploading appropriate documentation separately as PDF files
- Completion of mandatory training courses, as per Ontario Shores Centre for Mental Health Sciences requirements
- Payment of appropriate fees
Payment of MSA dues (as appropriate) for Active/Associate/Temporary staff:
